CHIP - Model notification letter
Even though employer groups are not required to notify employees of special enrollment rights, due to CHIP or Medicaid loss, until 2010, some groups may still wish to communicate sooner. A model notice letter, constructed by Blue Cross Blue Shield of Minnesota, has been posted to our CHIP resources page for your use.
